With increasing case load of chronic diseases like diabetes, hypertension, stroke , heart diseases etc regular monitoring plays important role in disease management and prevention of complications. This platform helps doctors monitoring their patients vitals and reports remotely enabling data based treatment plans and convenient and connected care for patients
Problem Statement
Increasing numbers of chronic diseases and their complications requires regular and connected monitoring. Doctors face challenges as they don't get data for better disease management. Patients deteriorates as no one is monitoring health in between clinic visits and lands up in complications like stroke.
